If all the children purchase their own flats within 4km of their parents, they are able to benefit from the proximity grant. However, the parents do have to verify it on the HDB site during the purchasing process :)

Yes, this is possible, however, the parents will need to keep to the proximity rule during the minimum occupation period after the resale flat purchase.

Yes it is possible to use the same parents name for the PHG. Do note they cannot move out of the address for the next 5 years.
